Liaison Nurse

Status : Full-time

Office Location: Worcester, MA


Overview of Liaison Nurse Position:
Coordinates and facilitates referrals to VNA Care based on home health care and hospice admission criteria; recommends alternate plans of care as appropriate. Serves as on-site resource to assist facility staff that refers patients to VNA Care.
Bachelor of Science Degree in Nursing preferred or Graduate of Diploma or Associate Degree Program in Nursing.
Licensed to practice in Massachusetts.
Minimum of three years of clinical experience with one year in home health or hospice care preferred.
