CNA - New Horizon's Assisted Living

Status : Per diem

Office Location: Marlborough, MA

Description:

Position Summary:  Provides personal care services to Assisted Living facility residents. 
 
Functions & Responsibilities:
  • Provides personal care services and assists residents with full or partial baths, showers or whirlpools and care of the hair, nails, teeth and any other personal care needs as identified in service plan.
  • Observes and reports any variation in the resident's appearance such as pallor, flushed skin, decline in mobility, etc.
  • Reminds residents to take prescribed medication as reviewed and approved by the RN as stated in the service plan. Follows SAMM medication reminder guidelines and training and in compliance with EOEA regulations.
  • Keeps resident's room neat and clean and,in some cases, does routine light housekeeping.
  • Follows all assigned activities on the resident's service plan and reviews any questions or issues with the Charge Nurse/Wrap-Around Nurse.
  • Maintains strict confidentiality of resident information at all times.
  • Assures the safety of residents at all times.
  • Does laundry as necessary for the care and cleanliness of the residents. 
 
Education:
  • High School graduate or GED are required.
  • Certification from an approved Home Health Aide training program with skills checklist signed by RN required. 
 
Experience:
  • Previous hands-on patient care experience preferred.
  • Proficient verbal and written communication skills consistent with job requirements.
  • Reliable means of transportation. 
 
Schedule: All schedules available, over night, weekends, day, evening
