Hospice Medical Director - Woburn

Status : Full-time

Office Location: Woburn, MA

Description:

Position Summary: Responsible for the medical component of patient care.

Reports to: CEO

Functions & Responsibilities:

  •      Reviews clinical information for each Hospice patient: primary terminal condition, related diagnoses (if any) current subjective and objective medical findings, current medication and treatment orders and information about the medical management of any of the patient's conditions unrelated to the terminal illness.
  •         Provides written certification that it is anticipated the patient's life expectancy is 6 months or less if the illness runs its normal course within established Network policies and procedures.
  •        Performs an admission history and physical for patients who have no other physician.
  •      Maintains liaison with the patient's attending physician, encouraging the patient's attending to provide primary care to his/her patient in collaboration with the interdisciplinary team.
  •       Assists in developing the Plan of Care for each patient/family with the coordination of the patient's physician.
  •      Attends and actively participates in interdisciplinary team meetings.
  •          Maintains 24 hours, 7 days per week medical coverage when attending physicians or physicians designated to act in the attending physician's absence are unavailable.
  •         Participates in establishing written, programmatic guidelines for symptom control (e.g. pain, nausea, vomiting or other symptoms).
  •      Participates in hospice performance improvement (PI) activities.
  •       Available to perform Palliative Care Consults/Assessments in the home, in a hospital and in skilled nursing facilities.
  •        Provides medical oversight to Hospice Physician, Hospice back up physicians and nurse practitioners.
  •       Represents the values and mission of the Hospice program among physician colleagues and the community at large
  •       Educates community physicians on the principles of Hospice and Palliative medicine.
  •       Upholds the compliance objectives and policies and procedures of the Foundation and Subsidiaries.

Education: 

  • M.D. or D.O
  • Licensed to practice in Massachusetts.
  • Hospice and Palliative Care certification preferred.

Experience Preferred:

  • Minimum 3 years of medical practice.
  • Experience in hospice and/ or palliative care.
  • Experience in supervising other Hospice physicians and nurse practitioners.
